If you are contacting us regarding a new application or revalidation application submitted via the Provider Application and Validation for Enrollment (PAVE) portal on or after October 31, 2022 or would like more information on how to enroll in the Medi-Cal Dental Program, please contact Provider Enrollment Division (PED) using the Inquiry Form found on PED's website under 'Provider Resources' at https://www.dhcs.ca.gov/provgovpart/Pages/PED.aspx or via email at PAVE@dhcs.ca.gov. If a Medi-Cal Dental Fee-For-Service provider is interested in becoming a Dental Managed Care (DMC) provider, providers may contact our DMC plans as follows:
